What is the Oregon Health Plan (OHP)?
The Oregon Health Plan (OHP) is a program that pays for low-income Oregonians' healthcare. The State of Oregon and the US Government's Medicaid program pay for it. It covers different groups of services, called benefit packages.
Benefits
OHPcovers doctor visits, prescriptions, hospital stays, dental care, mental health services, and help with addiction to cigarettes, alcohol and drugs. OHP can provide glasses, hearing aids, medical equipment, home healthcare, and transportation to healthcare appointments.

Do I qualify?
Trillium does not determine whether or not you qualify. For Oregon Health Plan eligibility information, please visit Oregon’s online eligibility system to find out if you qualify for OHP and other state assistance programs.
How do I apply?
Fill out an online OHP application attheOregon Eligibility website completely and carefully. If you have problems answering all of the questions, please call us at 1-541-485-2155 and we will try to help you. If you don't have access to the internet you can ask for a paper application by calling OHP at 1-800-699-9075 or TTY 711, or call your case manager if you have one. You can also download an application from the Oregon Health Authority websiteand fax it to: 503-378-5628 or mail the application to: OHP Customer Service, P.O. Box 14015, Salem, OR 97309-5032.
Better Care for More People
As of July 1, 2023, people of all ages who meet income and other criteria qualify for full OHP benefits and other services and supports, no matter their immigration status. This program is known as “Healthier Oregon.” To find out more, visit the Oregon Health Plan website.
