Nothing is more important than you and your family’s good health. Harbor Freight Tools recognizes that you have different needs when it comes to your medical plans. Depending on where you live, you have as many as four medical plans to choose from.
- Anthem Blue Cross High Deductible PPO Plan with Health Savings Account (HSA)
- Anthem Blue Cross PPO Plan
- Kaiser or Anthem Blue Cross HMO Plan (California Only)
- Anthem Blue Cross Select HMO Plan (California Only)
Some plan features are standard. For example, all plans cover preventive care at no cost to you, all offer coverage for prescription medications and all provide financial protection in the event of a major illness or injury.
Other features may be unique to a particular plan. Picking the right plan depends on your healthcare needs and budget.
Anthem High Deductible PPO w/ HSA
The Anthem Blue Cross High Deductible PPO plan gives you the freedom to seek care from the provider of your choice. You will maximize your benefits and reduce your out-of-pocket costs if you choose a provider who participates in the Anthem Blue Cross network.
With this plan, you are able to open a health savings account (HSA), which allows you to save pre-tax* dollars to help pay for qualified health care expenses like medical care, prescription drugs, and dental and vision care.

Here’s how the High Deductible PPO works:
Deductible
- You must meet the plan’s deductible before if begins to pay for most medical care and prescription drugs
- If you enroll family members, one or more members must meet the fullfamily deductible before the plan begins to pay for any individual
- In-network preventive care and some generic preventive medications are fully covered without having to meet the deductible
Coinsurance
- Once you meet the plan’s deductible, you are responsible to pay a percentage of your medical expenses, called coinsurance
Out-of-Pocket Maximum
- Once your out-of-pocket costs (deductible + coinsurance) meet the plan’s out-of-pocket maximum, it will pay 100% for all covered services for the rest of the plan year
- If you enroll family members, one or more members must meet the fullfamily out-of-pocket maximum before the plan pays at 100% for any individual
Health Savings Account (HSA)
- Set aside pre-tax* dollars through payroll deductions to help pay your deductible, coinsurance, and any other out-of-pocket health care costs
- To help you save, Harbor Freight Tools also contributes to your account:
- You contribute at least $5 per paycheck: Harbor Freight Tools annual contribution of $500 employee / $1,000 family
- You don’t contribute your own funds: Harbor Freight Tools annual contribution of $250 employee / $500 family
- All Associate and company contributions are prorated over 24 pay periods each year (52 pay periods for Associates in New York and Rhode Island)
- To be eligible to participate in an HSA, you must:
- Be covered by an HSA-compatible high-deductible health plan, and no other plan (including Medicare or TRICARE)
- Not be claimed as a dependent on another person’s tax return (not including your spouse)
The guidelines for contribution maximums are set by the IRS each year. The maximum amount that can be contributed into an HSA (including Harbor Freight’s contribution) is outlined in the table below:

Anthem PPO
The Anthem Blue Cross PPO plan gives you the freedom to seek care from the provider of your choice. You will maximize your benefits if you choose a provider who participates in the Anthem Blue Cross network.
Your in-network physicians will submit claims for you. If you receive treatment from an out-of-network physician, they may require you to pay the entire amount at the time of service and submit a claim for reimbursem*nt.
The calendar year deductible must be met before certain services are covered.

Anthem HMO (California Only)
With the Anthem Blue Cross HMO plan, you select a primary care physician (PCP) from the participating network of providers, who will coordinate your health care needs, including referrals to specialists and approving further medical treatment. Services received outside of the HMO network are not covered, except in the case of emergency medical care.

Anthem Select HMO (California Only)
With the Select HMO plan, you will still receive the same high quality care as the Full HMO. Like the Full HMO, you still must select a primary care physician (PCP); however, this plan has a slightly more limited network of providers, which means you will pay less for your medical care!

Kaiser HMO (California Only)
We are excited to introduce to you our new Kaiser HMO medical plan for California Associates. You must be located within 30 miles of a Kaiser facility to enroll in this plan. Medical Costs
For most Associates, the 2024 plan year has 26 pay periods; however, only 24 paychecks will have deductions for Associate benefit contributions. Associates in New York and Rhode Island will be paid on a weekly basis. To calculate your weekly deduction, multiply the premiums by 24, then divide by 52.

Machine Readable Files (MRFs)
Transparency in Coverage (TIC) regulations require health insurers and group health plans to create machine readable files (MRFs) that contain the negotiated rates for in-network providers and allowed amounts derived from historical claims for out-of-network providers and make those files publicly available. A machine readable file is a digital representation of data or information in a file that can be imported or read by a computer system for further processing without human intervention.
