Win/Loss statements are available online and through the mobile app. They are available the first week in February for the previous year. You will be able to access and print the past five year's win/loss statements. To access this information you will need to login to your My Rewards account, select "Win/Loss Statement", check: “I Accept the Terms and Conditions” and click Generate Statement.
Your Win/Loss Statement will launch in a new window - if it does not appear please be sure you have enabled pop-ups in your browser. It may take up to 30 seconds to generate your report.
If you haven't set up your online account yet, it only takes a moment. All you need is your valid Caesars Rewards account number, your date of birth, and Zip Code. Please click here to create your online account.
If you need a Win/Loss statement from a Caesars Rewards Local destination, you will need to visit that property’s Caesars Rewards Center or click hereto request it online.

If you need to fill out a manual form, see attachment. If you don't have your Caesars Rewards account number, simply fill out the form as much as possible. Requests will not be processed until after the first week in February.
